Training Registration

Please fill out and submit the following registration form. Instructions for completing your registration will appear on the next page.

Cost:

Total training cost is $1,500. $750 is due at the time of registration, and the balance is due two weeks prior to the start of the training.

Cancellations made one month or more before the first day of training will be completely refunded. Later cancellations will only be refunded in full if your space is filled; otherwise, we will retain a cancellation fee of $200.

Make sure the name you used for your registration is indicated clearly on your check, along with the training dates you are registering for. If you have any questions, please contact Diane Neumann at 617-926-9100.
