Please fill out and submit the following registration form for the October training. Call (888) 236-CCDM to register in the May training in Connecticut. Instructions for completing your registration will appear on the next page.

Location:	Wellesley College Club
Cost:	Total training cost is $1,500. After your $200 deposit the remaining payment of $1,300 is due six weeks before the first day of training. You will receive confirmation, directions and agenda after we receive your registration fee. Cancellations made one month or more before the first day of training will be completely refunded. Your deposit for later cancellations will be returned only if the space is filled.
